Intermountain Healthcare’s (IHC) Gene Smith, MHA, MBA, Operations Director, SDoH, has been at the forefront of developing innovative solutions for Social Determinants of Health (SDoH) matters in the healthcare setting. IHC’s Alliance for the Determinants of Health led to the creation of a referral platform, mobilized community health workers, distributed discretionary funds, and built a community infrastructure. Through this program, IHC has learned key takeaways such as how to utilize predictive analytics, develop partnerships, and comply with HIPAA to ensure that patients are connected to the resources they need.
