Sandra Dieleman is President and CEO of Avera@Home, a not-for-profit company offering comprehensive home-and community-based services including home health, care transitions, hospice, private duty, home medical equipment and faith community nursing. A part of Avera Health, Avera@Home serves patients across eastern South Dakota, Minnesota, Iowa and Nebraska.

Dieleman launched Avera@Home as a start-up organization in 2013; under her leadership the average daily census for both home health and hospice has more than doubled, and Avera@Home’s geographic coverage area has grown so that nearly all patients in the Avera footprint have access to services. Avera@Home’s scope and reach across the region made it possible to launch a COVID-19 virtual program during the COVID-19 pandemic to care for moderately ill and high-risk patients; the program ultimately served over 5,000 patients in their homes, creating capacity for the regional hospitals in the system to care for the most severely ill patients.

Dieleman came to Avera from UnityPoint Health where she served as the Chief Financial Officer for UnityPoint at Home.
