As a safety net hospital with limited resources\, the organization depends on foundation funding and grants to support outreach programs\, making reliable data essential for informed decision-making on resource allocation. Data analytics in the EHR allow Humboldt Park to measure progress in key areas such as hypertension\, diabetes\, substance use disorder\, and women’s health while incorporating Social Determinants of Health (SDOH) data across the entire care continuum.
