Humboldt Park Health\, located in Chicago\, IL\, is proud to be the first organization in the Midwest to earn a Healthcare Equity Certification from The Joint Commission\, a distinction held by only 17 other hospitals nationwide since The Joint Commission created the program in July 2023. Serving nearly 500\,000 residents\, 40% of whom identify as Hispanic/Latinx and 28% as Black/African American\, Humboldt Park Health is deeply committed to addressing health disparities through inclusive and culturally competent care. As a safety net hospital with limited resources\, the organization depends on foundation funding and grants to support outreach programs\, making reliable data essential for informed decision-making on resource allocation. Data analytics in the EHR allow Humboldt Park to measure progress in key areas such as hypertension\, diabetes\, substance use disorder\, and women’s health while incorporating Social Determinants of Health (SDOH) data across the entire care continuum. This approach allows care teams to assess how factors such as housing\, food security\, employment\, and education are impacting patient health.
